Participant Agreement & Medical Waiver

This Agreement is between you ("Participant") and Odyssey 45 LLC, a California limited liability company ("Odyssey 45"), and the facilitators of the Cost of Caring workshop — Jon Rose and Stacia Butterfield (the "Facilitators"). By submitting your participation form and payment, you agree to all of the terms below.

01Description of the workshop

Cost of Caring is a small-group, multi-day workshop combining facilitated discussion, group practice, written reflection, and Grof-style holotropic-tradition breathwork led by Stacia Butterfield, a certified GTT (Grof Transpersonal Training) facilitator. The workshop is non-clinical, non-therapeutic, and is offered for educational and personal-development purposes only.

02Voluntary participation

You acknowledge that your participation is entirely voluntary. You are free to pause, step out, or withdraw from any element of the workshop at any time without penalty. You also acknowledge that you may at any point be asked by the Facilitators to step out of a session — including the breathwork portion — if they reasonably determine that continuing is not safe for you or the group.

03Assumption of risk

You understand that conscious-connected and holotropic-style breathwork can produce strong physiological and psychological responses, including but not limited to:

• Lightheadedness, tingling, muscular contraction (tetany), and altered states of consciousness
• Emotional release, including grief, anger, fear, and uncontrolled crying or laughter
• Surfacing of memories or material that may be unfamiliar or distressing
• Physical fatigue, headache, or temporary disorientation following sessions

You voluntarily assume all risk of participating, including risks not specifically listed above. You understand that breathwork is not a substitute for medical or psychiatric treatment.

04Medical and psychological representation

You represent and warrant that the medical and psychological information you provided on your participation form is true, complete, and accurate to the best of your knowledge. You agree to notify the Facilitators in writing of any material change in your medical or psychological condition between the date of your registration and the workshop start date.

You acknowledge that misrepresentation or material omission on the participation form is grounds for immediate dismissal from the workshop without refund.

05Contraindications you have disclosed against

You acknowledge that the following conditions are contraindications for the breathwork portion of this workshop, and that by participating you are representing that none of them apply to you, OR that you have disclosed them on your participation form and have been cleared by Stacia Butterfield specifically:

• Current pregnancy
• Cardiovascular disease, severe hypertension, or recent cardiac event
• Epilepsy or seizure disorder
• Detached retina, glaucoma, or recent eye surgery
• History of psychosis, schizophrenia, bipolar I disorder, or borderline personality disorder
• Active substance dependence or untreated addiction
• Major surgery in the past 90 days
• Severe osteoporosis or recent bone fracture
• Current psychiatric medications without prior clearance from your prescribing provider

06Not medical or mental-health treatment

You understand that neither Odyssey 45 LLC nor the Facilitators are providing medical advice, psychotherapy, diagnosis, or treatment. Nothing said or done during the workshop is intended to diagnose, treat, cure, or prevent any condition. If you are under the care of a medical, psychiatric, or psychological provider, you agree to consult with them prior to attending and to continue any prescribed treatment.

07Confidentiality of the group container

You agree to keep the personal stories, identities, and disclosures of other participants confidential. What is shared in the workshop stays in the workshop. You are free to discuss your own experience publicly; you are not free to discuss anyone else's.

08Photography, recording, and content

No participant may photograph, audio-record, or video-record any portion of the workshop without the express prior written consent of the Facilitators and any participant who would appear in the recording. Odyssey 45 may, separately and with advance notice, photograph or record the venue, materials, or Facilitator-led moments for documentation; no participant will be identifiably included without their consent.

09Right of dismissal

The Facilitators reserve the right to dismiss any participant whose conduct disrupts the workshop or endangers themselves or others. Dismissal under this section is without refund.

10Release and waiver of liability

To the fullest extent permitted by California law, you, your heirs, and your representatives release and forever discharge Odyssey 45 LLC, its members, Jon Rose, Stacia Butterfield, the venue, and their respective agents and employees from any and all claims, demands, actions, or liabilities arising from or related to your participation in the workshop, including any injury, illness, emotional distress, or property damage, except in cases of gross negligence or intentional misconduct.

You understand that this release covers risks that are inherent to the workshop activities, and that you have read and understood the Assumption of Risk section above.

11Refunds

Refunds are governed by the Cost of Caring Refund Policy, which is incorporated by reference into this Agreement.

12Governing law

This Agreement is governed by the laws of the State of California. Any dispute arising under this Agreement shall be resolved in the state or federal courts located in Placer County, California, and you consent to the jurisdiction of those courts.

13Entire agreement

This Agreement, together with the Refund Policy and your completed participation form, constitutes the entire agreement between you and Odyssey 45 LLC regarding your participation in the Cost of Caring workshop and supersedes all prior discussions or representations.
